Important Information:

Screenings (Hearing and Vision) - completed annually for grades: PreK, K, 1 (by Nov. 1st), 3, 5, 7, and 9. Any student who does not pass the standards set forth by The Ohio Department of Health will receive a written letter explaining the need for a referral. Please keep in mind, this is a reference tool, not a diagnosis! For example, if your child is referred for failing their hearing test, it doesn't mean they can't hear, they may just have an ear infection or sinuses.

Medications - If your child needs to take any prescription medication during school hours, the school must have the Prescription Medication Authorization Form completed and on file prior to administration. This is required for any and all prescription medication so make sure before you leave the doctor's office to get the form signed or have the doctor provide the school with written documentation for permission for us to administer at school.

Your child should stay home from school if they have any of the following symptoms:

-Vomiting and/or diarrhea

-Fever >100 degrees (your child must be fever free without medicine for 24 hours)

-Cough with colored nasal discharge

-Itchy or red eyes with drainage

-An unusual rash

-Live head lice

-If your child has a highly contagious illness (Pink Eye, Strep Throat, etc.) they must be on antibiotics for 24 hours prior to returning to school

Head lice - is always a concern in schools because anyone can get lice from direct hair to hair contact or by sharing personal items or clothes. Head lice is a nuisance, not a communicable disease! Check your child’s hair regularly. If you suspect lice, ask your health care provider to recommend appropriate treatment. Inform the school nurse that your child has head lice. Your child may not return to school until he/she has been treated and all live lice are removed. A parent must accompany the child back to school, and have the school nurse or front office check their head. If your child has nits (lice eggs), you may receive a written letter informing you.

Immunizations - Your child will need immunizations prior to PreK, K and 7th grade. Please contact your primary care provider or health department for any questions regarding vaccinations. I am required to send immunization data annually to The Ohio Department of Health. ODH may audit our immunization records at any time throughout the school year and if your child is not up to date, may exclude your child from school until their shots are up-to-date. If your child's immunizations are not up-to-date, please get their shots ASAP and remember to give the school an up-to-date copy of your child's immunization record each time they receive a vaccination.
